For paper assessment deployments, Account Setup Form (ASF), W-9 and Signature Log submissions for the 2026 IOA Program, please note the following updates to return modalities:

• Email submissions are replacing Optum Uploader.
  Please utilize the new email address: ioa@optum.com.
• Our fax number has been updated to: 1-877-273-3868
• Our physical mailing address has been updated to:
  Optum Prospective Programs Processing,
  1 E. Washington St., Ste. 900, Phoenix, AZ 85004

All program materials supporting the above submissions, including but not limited to the IOA Program instructions documents, paper assessments, ASF and Signature Log, have been updated to reflect the updated submission options.

• If needed, you may request the updated ASF and Signature Log from your Optum representative. (Note: These documents will no longer be included in printed assessment orders.)
• Please note, if no changes are needed to your setup for 2026, you do not need to submit a new ASF, W-9 and/or Signature Log.
